    Who qualifies for Home Health Services?

      • Any patient that has a need for skilled services in the home such as nursing, physical therapy or occupational therapy and under the care of a physician.
      • The patient must be homebound if he or she is a Medicare patient. This is defined by Medicare as "Considerable and taxing effort to leave home, infrequently, and for short durations of time."

Our Staff and Services

      • Skilled Nursing- our licensed nurses perform wound care dressing or change and, teach family care. They place and care of tubes for feeding, catheters and performs intravenous therapy. They inject medications and perform diabetic care and instructions in all phases of diabetes.Our skilled nurses also teach patients about monitoring new medications and remove fecal impactions and initiating bowel and bladder training program.
      • Physical Therapists- our licensed physical therapists offer ambulation training, use of equipment and prosthesis, and increase mobility. A patient who has unstable gait and has a tendency to fall and lose balance is a good candidate for Physical Therapy service.
      • Occupational Therapists- our licensed occupational therapists work to improve upper body strength and mobility, train in home and self care activities. A patient with decreased ability to take care of themselves due to physical or psychological limitation is a good candidate for Occupational Therapy service.
      • Speech Therapists- our licensed speech therapists offer such services to increase comunication following a trauma and to improve swallowing techniques. A patient that had a stroke and has problem with communication and swallowing will be a good candidate for speech therapy service.
      • Medical Social Workers- help to alleviate stresses encountered during illnesses, and provide helpful information and support to assist in long term planning. A patient that cannot cope up with his/her illness due to some emotional or environmental factors could be a good candidate for Medical Social Worker evaluation.
      • Personal Assistance Aides- our Home Health Aides assist with bathing, hygiene tasks changing of bed linens and general tidying of the patient's area. A patient that has physical limitation to perform these tasks (bathing, hygiene, bedding or general tidying) is a good candidate for Personal Assistance service.
