Team Effort:
After
receiving approval from a physician, CARE EXCELLENCE HOMEHEALTH
will send a registered nurse to evaluate and assess the patient's
medical condition, initiate a plan of care and assign necessary
disciplines. Physical, occupation, speech are part of our rehabilitation
team and administer therapy according to patient's need.

CARE EXCELLENCE HOMEHEALTH, we strongly believe that educating
patients and families promotes dignity and self-reliance. Our
staff also includes a medical social worker who addresses the
emotional, financial and psychosocial needs of the patient and
links the patient to community resources. All of these services
are available under the direction of a physician and are supervised
by a registered nurse.
